Hmmm.
Well, I have this one idea: there's a group of people who are not connected to each other at all - at least they think so.
And then, one day, they start having this hallucinations/visions, whatever, when they go into a tribal setting. (Yeah, it would probably be better than medieval. xD) And it happens to all of them at the same time.
And there they either die all the time, or some of them die, or something.
And the explanation (or the basics of it at least, haha xD;) could be that there was some sort of ritual that bound a bunch of people together (for some reason) and now their reincarnated versions (that would be our players) need to figure out why.
