A Wonderful Life has its chapters span three years
Another has them down only one due to player complaints of three chapters being far too long
But for the most part, the big difference is your suitors
Some have the option to choose boy or girl tho
And special additions generally see one boy/girl normally close but not quite eligible get aged up
Some smaller differences are that some useless items in the earlier of the two released (always the boy version, as I recall) get some minor use in the girl version, and sometimes the intro is different. There's some minor text changes, too

Friends is all sorts of messed up in the text, the mirror is a requirement to marry a girl you choose, and the pond's just for filling the watering can even tho there's a river that wrapped around 1.5 sides of the farm. Intro is mayor getting mad cuz he assumes you're there to cause a mess at the farm, the old guy who owned it died. Well, it turns out that you're a friend of said guy, have been since you were a kid and your family vacationed at Mineral Town and you ran off on them and ended at the farm. You've been willed the farm, since the guy died six months (so in game, a year and a half) ago, so it's kinda messy from neglect
More Friends has the mirror have the option of changing overalls color, the pond can store fish, except for the occasional derp here and there, the text is all fixed, you get the girl suitor scenes as friends on top of the new boy ones, and the intro. You live in the city. Tired of the same thing every day. Literally a drag. So look at the newspaper and go to bed. Nope, see an ad for an amazing farm, get excited, call, buy farm, quit job, move out...And the farm's a neglected mess. Mayor laughs at you once he realizes who you are: another suckered person. So you get mad. Game automatically has you hit him with the hammer once, for which he apologizes. You can, however, not accept multiple times and abuse him with hammer, axe, watering can, hoe, sickle, and I believe that's it, I don't think there's a sixth option
But once you forgive him, he does admit that the house, though small, is still in good shape, so hey, since you ditched city life entirely, why not give it a shot? If you say no, game over, credits roll. If you say yes, game on
And for a cutscene difference, Elli's Black Heart event is a noticable one. You visit the hospital. Suitor scene music plays. Elli's in front of the counter. Her...Little brother? Nephew? Can't remember, but his name is Stu. Stu tells Elli that he has something. She asks to see. He shows her. It's a bug, she shrieks, Stu laughs. He's about six, kids that age can be jerks. Then you walk up, and Stu hides behind you. If you've not spoken to Elli yet, y'all get introduced. If you have, "Oh hello Kit, are you feeling all right?" or something to that effect. You ask what's going on, Elli tells you, calling out the kid. He says he thought she might have liked that one, you know I hate bugs is the retort
Aw, c'mon! Stu turns to you. You understand me, right?
Boy: after all, you're a guy like me!
Girl: after all, you're young at heart!
Saying yes gets Stu excited and Elli warns you that he'll start doing it to you (he doesn't). Saying no gets Stu crying, and he runs off. Elli is somewhat irked by his behavior, but is relieved cuz she thinks he'll stop doing it now. You don't find out either way
