I think that declaration is only in case something happened to her and she wanted to sue them. If she lied in the form, she can't really do that, it's on her if anything happens.
Unless it's faulty device causing the injury, then maybe.

i broke my pinkie once and it wasn't fun

That's painful! my pinkie was painful too so i can imagine an arm is worse

I broke my pinky playing basketball with bf and some kids last year, that's the only bone I ever broke in my life XD At first I thought it might be just out of its socket but it graually hurt more and more so in about 5-10 from breaking I began crying and we had to go to the hospital. Then, I was laughing all the time and bf was all remorseful and sorry because I broke the pinky from his throw to me and I'm like duude, stahp, it's nobody's fault XD And he was like "your finger is broken, how can you smile so much? "
